How do I make an appointment?

To schedule an appointment you must first complete the online application.

Please send us your records or complete the medical record release form to allow us to request your records. Once we receive your records and review them for your pre-approval can schedule an appointment to see our physician.

This will ensure that you qualify for the Oregon medical marijuana program for you schedule an appointment. There will be a $40 appointment deposit due at the time of scheduling. This fee will be applied to the total cost to see the physician. The remaining balance of $155 will be due at your appointment.
