How long after my appointment do I have to submit the provided application packet?

You have up to 90 days after the physician signs your attending physician statement submit your state application.

If you submit your application after the three month window, it will be rejected in you will have to see the physician again. In the event that this happens, you will be required to pay the physician fee again, as we will need to complete the same process over again.
